2. Log in to your account and choose the application form that says "Issuance of Survival Member Certificate". Fill in all the mandatory information and upload a scan copy of all original documents. When you've completed the form in a timely manner, acknowledgement is created.

3. Print a acknowledgement in its entirety and include all original photocopies along with it. keeping the originals with you to verify the papers at the SDM office.

4. Form filling and verification procedure completed after proper verification for Data The Certificate is issued on the internet. With the Acknowledgment number. the applicant is able to access an electronic copy of the Survival Member Certificate.
